History
Cochlear’s history is closely connected with the development of the modern multichannel cochlear implant in Australia. Professor Graeme Clark pursued the technology after observing the effects of hearing loss in his family and working on the engineering and clinical problems involved in stimulating the auditory nerve. His research helped demonstrate that an implant using multiple electrodes could provide useful auditory information rather than merely producing a single sensation of sound. In 1981, Nucleus, an Australian medical-electronics company, established Cochlear as a subsidiary to commercialize the technology. The project received Australian government financial support and brought together medical researchers, engineers, manufacturers, clinicians, and regulators. The first commercial Nucleus cochlear-implant procedure took place in 1982. This early commercial achievement gave the company a foundation in both product development and the specialized clinical infrastructure required for implantation, programming, rehabilitation, and long-term support. The Nucleus 22, also known as the CI22M, became an important regulatory milestone when it received United States FDA approval in 1985 as the first FDA-approved multichannel cochlear implant. The Nucleus L24 Hybrid implant received FDA approval in 2014 for adults meeting the relevant candidacy criteria. Cochlear expanded beyond conventional cochlear implants in 2004 through the acquisition of Entific Medical Systems, a Swedish company associated with bone-anchored hearing systems. The transaction added the Baha business and broadened Cochlear’s addressable market to include conductive and mixed hearing loss and certain cases of single-sided deafness. The company also manufactures auditory brainstem implants, which bypass the cochlea and stimulate the cochlear nucleus in the brainstem for patients who cannot use a conventional cochlear implant. External sound processors evolved from early body-worn analog equipment to smaller digital and wireless devices. In 1995, some CI22M implants were recalled because of internal power-supply failures associated with incorrectly installed capacitors; fewer than ten devices reportedly required explantation. In September 2011, Cochlear voluntarily recalled the CI500 implant family after reports of shutdowns or “soft failures.” The recall affected a strategically important product range, led the company to pause production, and caused a substantial fall in its share price. Cochlear returned to the CI24RE platform while investigating the cause and developing replacement products. Cochlear also faced a US compliance case involving its Partners Program. A whistleblower complaint alleged that the program offered credits, discounted products, gifts, travel, and other benefits to physicians and providers in connection with the use of Cochlear devices. In 2010, Cochlear Americas paid US$880,000 to resolve allegations under the Anti-Kickback Statute and False Claims Act. The matter became a prominent example of the regulatory risks associated with commercial activity in implantable medical devices. In 2022, Cochlear agreed to acquire Oticon Medical’s hearing-implant business from Demant. The original proposal covered a wider business, but competition authorities raised concerns about the effect on competition and innovation, particularly in bone-conduction systems. The transaction was narrowed to the cochlear-implant business and completed in May 2024. Cochlear accepted responsibility for supporting approximately 20,000 existing Oticon Medical cochlear-implant recipients, integrating them into its long-term support infrastructure.
