Its distribution into lung and immune cells delivers far higher localized drug concentrations than erythromycin, with antibacterial activity 6 to 10 times stronger against common Gram-positive respiratory pathogens, and an overall adverse reaction rate as low as 3% that is far better tolerated by patients.

Clarithromycin is a first-line recommended prophylactic and treatment drug for Mycobacterium avium complex (MAC) infections in immunocompromised people living with HIV. It also has documented mild anti-inflammatory and immunomodulatory properties that have made it a standard long-term maintenance therapy for diffuse panbronchiolitis, a rare chronic lung condition first identified in East Asian populations.

Overprescription of clarithromycin for trivial unconfirmed bacterial upper respiratory infections has driven rising resistance rates among key pathogens including Streptococcus pneumoniae in many global regions. Public health regulators now increasingly recommend restricting its empirical use, reserving its high efficacy specifically for priority indications such as confirmed H. pylori eradication, confirmed atypical pneumonia, and non-tuberculous mycobacterial treatment to extend the clinical lifespan of this important antibiotic.
